I hereby agree to my child representing EMC Academy and therefore accept the academies code of conduct for players and spectators. I agree to the following conditions:

1. I agree that subscription for Friday night training will be £200/£220/£350 and will be payable upon my child’s registration. This payment does not include playing kit or fee for EMC tournaments/Pro fixtures. (Subscription payment due no later than 07/07/19)

2. The Academy and its officials are not responsible for any accidents/injuries, whilst travelling to or from, or during matches, training or functions. If personal accident or personal property insurance is required, this must be provided on a personal basis.

3. I agree that my child will abide by the rules of the club and conduct themselves in an appropriate manner both on and off the field of play.

4. I understand that there is to be no individual coaching from the side-lines. This includes giving technical instructions whilst the game is going on, to any child. I agree to let them play with the freedom of making their own mistakes, and learning from them these experiences.

5. I understand that an EMC Academy kit must be purchased and worn to every training session and fixture and must be cared for in proper fashion.

6. I understand and agree to use WhatsApp Broadcast Messaging service as the main form of communication for all updates relating to EMC Academy. I acknowledge that the main mobile number I provide will be the only number used by the academy for communications, and that additional numbers linked to the player will not be included in communications.
